The Weekend Visit RN supports our family’s needs between the hours of 8am-5pm. The professional nurse is accountable to the Director of Patient Care Services for implementing the patient plan of care and communicating each patient needs to the Interdisciplinary team. The professional nurse is also responsible for communication and collaboration with community physicians as well as family members regarding individual patient care. Requirements

Patient Care

 The Hospice Team Nurse provides professional nursing care to patients by utilizing all elements of nursing process.

Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
