As a Licensed Practical Nurse (LPN) at Autumn Lake Healthcare at Alice Manor, you will have the opportunity to work alongside a talented and supportive team of healthcare professionals to provide high-quality care to our residents. You will be responsible for providing nursing care to our residents, including administering medications, providing wound care, and developing individualized care plans. You will also have the opportunity to participate in our interprofessional care team, collaborating with physicians, therapists, and other healthcare professionals to ensure the best possible outcomes for our residents.

Responsibilities:

• Provide high-quality nursing care to our residents, including administering medications, providing wound care, and developing individualized care plans

• Participate in our interprofessional care team, collaborating with physicians, therapists, and other healthcare professionals

• Maintain accurate and up-to-date records of resident care and services

• Monitor and report changes in resident condition to the nursing team

• Develop and implement individualized care plans that meet the unique needs and goals of each resident

• Participate in on-call duties as needed

Qualifications & Experience Requirements:

• Previous experience as a Licensed Practical Nurse (LPN) preferred

• Must have a Licensed Practical Nurse (LPN) License
