Position Overview

The Licensed Practical Nurse (LPN) provides direct care to residents, observes and monitors health and vital signs, maintains accurate documentation, and works closely with Registered Nurses (RNs) and Unit Managers to deliver safe, high-quality care.

Key Responsibilities

• Provide personal care and assistance with daily living activities

• Monitor resident health and report changes to nursing staff

• Collaborate with RNs and Unit Managers on care plans

• Maintain accurate and timely documentation in compliance with facility and state standards

• Support a positive, resident-focused environment for both residents and staff

Qualifications

• Current Licensed Practical Nurse (LPN) license in Massachusetts

• Long-term care experience preferred but not required

• Strong communication, teamwork, and problem-solving skills

• Compassionate, reliable, and committed to high-quality resident care
