Job Summary:

Our Valparaiso Porter-Starke location is seeking a dedicated and compassionate Nurse Practitioner to join our team of healthcare professionals. This position may be required to report to other organizational locations based on operational needs.

Responsibilities:

• Identifies, evaluates, and assesses the mental health needs of clients, developing treatment plans and prescribing appropriate interventions and medications.

• Works with individuals, families, and groups to assess emotional, social, and physical health needs, consulting with general medical professionals as needed.

• Documents all diagnoses, ensuring accurate documentation of disease progression and manifestation.

• Seeks consultation and supervision in complicated cases, evaluating clients' responsiveness to treatment and recommending changes in treatment goals.

• Provides coaching, instructions, and direction to professional and paraprofessional staff, participating in medical staff meetings and committee assignments as directed.

Education and Experience:

• Current licensure and prescriptive practice as a Nurse Practitioner.

• Experience or training in mental health and/or chemical dependency treatment is required.

Required Certification/License:

• Licensed nurse practitioner license is required.
