Responsibilities:

• Provide direct patient care under the supervision of licensed nursing staff, including vital signs, bathing, dressing, and feeding

• Assist patients with daily living activities, such as toileting, ambulation, and transfers

• Maintain accurate and detailed records of patient care and observations

• Collaborate with the interdisciplinary team to develop and implement individualized care plans

• Participate in patient orientation, education, and discharge planning

• Ensure a safe and clean environment for patients, families, and staff
