About the Role:

Campbellsville Nursing and Rehabilitation Center is a leading provider of skilled nursing care, dedicated to helping our residents recover, thrive, and reach their full potential. As an LPN/RN on our team, you will:

• Provide direct patient care, including administering medications, wound care, and other treatments as prescribed by physicians

• Collaborate with interdisciplinary teams to develop and implement individualized care plans

• Monitor residents' conditions, detecting and reporting changes to ensure timely interventions

• Educate residents and their families on self-care techniques and disease management

• Participate in quality improvement initiatives to enhance patient outcomes and satisfaction

Responsibilities:

• Deliver high-quality patient care, adhering to established standards and protocols

• Communicate effectively with residents, families, and healthcare team members

• Maintain accurate and detailed records of patient care and progress

• Participate in workplace safety initiatives and maintain a clean and safe environment

• Contribute to the development and implementation of care plans, protocols, and policies
