About the Role:

As a Social Worker at Kadima at Lititz, you will have the opportunity to make a meaningful difference in the lives of our residents. Your primary responsibilities will include:

• Conducting social histories and assessments to identify residents' needs and develop individualized care plans

• Providing emotional support and counseling to residents and their families

• Facilitating communication between residents, families, and healthcare professionals

• Coordinating with other departments to ensure seamless care transitions and effective problem-solving

• Participating in interdisciplinary team meetings to discuss resident care and well-being

Requirements:

• Bachelor's degree in Social Work or a related field

• 1+ years of experience in a healthcare setting

• Strong communication and interpersonal skills

• Ability to work collaboratively as part of a team

• Compassionate and empathetic approach to care
