Key Responsibilities:

• Provide daily personal care (bathing, grooming, dressing, oral hygiene, etc.)

• Assist with daily living activities based on the Plan of Care

• Observe and report patient conditions

• Support with meal prep, feeding, and light housekeeping

• Accompany patients to medical appointments and essential errands

Requirements:

• Valid New York State HHA Certificate

• Government-issued photo ID & Social Security Card (or Work Authorization, if applicable)

• Two reference letters

• Recent physical exam

• TB screening (Quantiferon preferred)

• Proof of flu vaccination

• Rubella & Rubeola titers

• Chain of Custody drug screening (within the last 6 months)
