Responsibilities:

• Provide direct patient care to individuals recovering from injury, surgery, or managing chronic conditions

• Develop and implement personalized care plans in collaboration with the multidisciplinary team

• Monitor and report patient progress to ensure optimal rehabilitation outcomes

• Conduct assessments, take vital signs, and administer medications as prescribed by physicians

• Collaborate with physical therapists, occupational therapists, and speech therapists to create comprehensive rehabilitation plans

• Communicate effectively with patients, families, and healthcare professionals to ensure seamless coordination of care

• Participate in staff development and education activities to stay current with best practices and industry developments
