Park Avenue Nursing Home in Meadville, PA, is seeking a compassionate, motivated, and dedicated PRN Physical Therapist (PT) to join our rehabilitation team!

This is a wonderful opportunity for a PT who enjoys working in a Skilled Nursing and Long-Term Care (SNF/LTC) setting and is passionate about helping residents regain strength, improve mobility, and maintain their independence.

What You'll Do

• Evaluate residents and develop individualized physical therapy treatment plans

• Provide skilled physical therapy services to residents in a SNF/LTC setting

• Help residents improve strength, balance, mobility, and functional independence

• Monitor progress and adjust treatment plans to support positive outcomes

• Collaborate with PTAs, nursing staff, and the interdisciplinary care team

• Educate residents, families, and caregivers on therapeutic exercises and safety

• Maintain accurate and timely documentation using Net Health and PCC

• Provide compassionate, high-quality care that enhances residents' quality of life

What We're Looking For

• Graduate of an accredited Physical Therapy program

• Current, active Physical Therapist license in Pennsylvania

• SNF/LTC experience preferred, but not required

• Experience with Net Health and PCC preferred; training can be provided

• Strong communication, clinical, and organizational skills

• A positive attitude and genuine passion for helping others

• Ability to work independently while contributing to a supportive team
