•RESPONSIBLE FOR DISCHARGE PLANNING

•MDS COMPLETION

•QUARTERLY PROGRESS NOTES

•EXCELLENT DOCUMENTATION

•PSYCHOTROPIC MEDICATION MEETINGS AND REVIEW OF GDRS

•ATTEND MEETINGS DAILY

•IDT MEETINGS AND CARE PLAN MEETINGS

•INTERACT WITH RESIDENTS, FAMILY MEMBERS, PHYSICIANS, HOSPICE AND HOME HEALTH REPRESENTATIVES

•WORK CLOSELY WITH THE MANAGEMENT TEAM

•COORDINATE ROOM CHANGES AS NEEDED

•NUMEROUS OTHER RESPONSIBILITIES
