About the Role:

As a Discharge Planner at The Pearl, you will play a vital role in ensuring our guests receive seamless transitions from our care setting to their next destination. You will collaborate with interdisciplinary teams, patients, and families to develop and implement individualized discharge plans, ensuring continuity of care and optimal outcomes.

Responsibilities:

• Coordinate discharge planning activities, including assessing patient needs, identifying potential barriers, and developing intervention strategies

• Develop and maintain relationships with patients, families, and healthcare providers to ensure informed decision-making and effective care transitions

• Collaborate with interdisciplinary teams to identify and address potential issues, and develop solutions to optimize patient outcomes

• Stay up-to-date with relevant regulations, policies, and guidelines affecting discharge planning, and ensure compliance with all applicable regulations

• Participate in quality improvement initiatives to continuously improve discharge planning processes and outcomes

• Maintain accurate and timely records of discharge plans and progress

Requirements:

• 1 year of experience in discharge planning or a related field

• High school diploma or equivalent required; degree in a related field preferred

• Strong communication, interpersonal, and problem-solving skills

• Ability to work in a fast-paced environment with competing priorities

• Certification in discharge planning or a related field (to be updated once available)
