The Monarch at Brooklyn Rehabilitation & Nursing Center is seeking an experienced, results-driven Admissions Director to lead our admissions department and help drive census growth. This is an excellent opportunity for a motivated healthcare professional with strong relationship-building skills, a passion for customer service, and a proven ability to convert referrals into successful admissions.

The ideal candidate will have experience in skilled nursing and rehabilitation, established relationships with hospitals and referral sources, and the ability to work collaboratively with clinical, marketing, and administrative teams to ensure a smooth admissions process.

Responsibilities

• Oversee and manage the facility’s admissions process from initial referral through resident admission.

• Drive census growth by developing and maintaining relationships with hospitals, discharge planners, social workers, physicians, managed care organizations, and other referral sources.

• Review incoming referrals promptly and coordinate with clinical and financial teams to determine admission suitability.

• Work closely with nursing, social services, rehabilitation, business office, and facility leadership to facilitate timely and appropriate admissions.

• Ensure prospective residents and families receive responsive communication, guidance, and support throughout the admissions process.

• Monitor referral activity, conversion rates, admission trends, and census performance.

• Develop and implement strategies to increase referral volume and improve referral-to-admission conversion.

• Maintain accurate referral records, admissions documentation, and required reports.

• Coordinate insurance verification, authorization requirements, financial information, and other admission-related details.

• Ensure admissions practices comply with applicable regulations, payer requirements, and facility policies.

• Identify barriers to admission and work proactively with the interdisciplinary team to resolve them.

• Provide regular updates to facility leadership regarding referral pipelines, admissions performance, and census opportunities.

Qualifications

• Previous experience in admissions, marketing, business development, or referral management within a skilled nursing facility, rehabilitation center, or healthcare setting.

• Proven ability to build and maintain strong relationships with hospitals and community referral sources.

• Demonstrated success in driving admissions, improving conversion rates, and supporting census growth.

• Strong knowledge of the skilled nursing and rehabilitation admissions process.

• Excellent communication, organizational, negotiation, and customer service skills.

• Ability to manage multiple referrals and priorities in a fast-paced environment.

• Proficiency with Microsoft Office and admissions or referral-tracking systems.

• Strong leadership skills and the ability to collaborate effectively with interdisciplinary teams.
