Requirements for Transportation Certified Nursing Assistant (CNA):

• Active New York Certified Nursing Assistant (CNA) certificate

• Valid New York Driver’s License

• At least 3 months of CNA experience in subacute care or a nursing home (clinical experience while going through a CNA program doesn’t count)

• Must be willing to work on the CNA floor when needed

Responsibilities of Transportation Certified Nursing Assistant (CNA):

• Accompany residents to medical appointments outside the facility

• Safely assist residents with transfers, including wheelchair and stretcher transfers

• Work on the CNA floor when needed

• Serve as a liaison between residents and the facility by providing and obtaining information as needed

• Ensure appropriate communication and follow-up after resident appointments

• Hand-carry required documents, including medical records and forms, as well as specimens

• Complete additional duties as assigned by the Appointment Coordinator and/or Director of Nursing
