We are seeking a dependable, personable, and detail-oriented Registered Nurse (RN) to join our growing team on a per diem basis. This position is ideal for an experienced nurse looking for a flexible schedule, supplemental income, and an opportunity to use their clinical knowledge outside of a traditional nursing environment.

This is an assessment-focused position. No bedside nursing, medication administration, or hands-on nursing care is required.

Key Responsibilities

• Conduct in-person initial assessments for prospective home care clients to evaluate their needs, preferences, functional abilities, and safety considerations.

• Perform periodic reassessments for existing clients and document changes in their care needs.

• Assist in developing and updating individualized non-medical home care service plans based on assessment findings.

• Communicate assessment results and recommendations to the care coordination and management teams.

• Educate prospective clients and families about available home care services and answer questions regarding care options.

• Maintain accurate, timely, and confidential assessment documentation in accordance with company policies and applicable regulations.

• Identify changes in client condition or needs that warrant referral to an appropriate healthcare provider or additional services.

• Represent CareBuilders at Home professionally and compassionately during home visits.

Qualifications

Required:

• Current, active, and unrestricted Registered Nurse (RN) license in North Carolina or a valid multistate license authorizing practice in North Carolina.

• Strong assessment, communication, and documentation skills.

• Ability to work independently and manage appointments efficiently.

• Reliable transportation and willingness to travel to clients’ homes throughout the Raleigh area.

• Valid driver’s license and automobile insurance when using a personal vehicle for work-related travel.

• Strong interpersonal skills and a compassionate, client-centered approach.

• Ability to complete required documentation and communicate effectively with the office team.

Preferred but Not Required:

• Experience in home care, home health, geriatrics, case management, or community-based nursing.

• Previous experience conducting client assessments or developing care plans.

• Sales, business development, or client relationship management experience.

• Comfort discussing home care services with prospective clients and families.

Schedule & Flexibility

• PRN / per diem position with flexible scheduling.

• Assessments scheduled based on client needs and nurse availability.

• No fixed weekly schedule or guaranteed number of hours.

• Ideal for nurses seeking supplemental income or greater work-life balance.

• Local travel required throughout Raleigh and surrounding communities.
