Berkeley Health and Rehab is seeking a compassionate, dependable, and dedicated Licensed Practical Nurse (LPN) to join our nursing team! Responsibilities

• Provide direct nursing care to residents in accordance with physician orders, care plans, and facility policies.

• Administer medications and treatments as prescribed.

• Monitor residents' health conditions and report changes to the appropriate nursing personnel.

• Perform nursing assessments and document care accurately and timely.

• Assist with developing, implementing, and updating resident care plans.

• Communicate with residents, families, physicians, and other healthcare professionals.

• Supervise and support Certified Nursing Assistants (CNAs) in providing quality resident care.

• Maintain compliance with infection control standards and applicable regulations.

• Promote residents' dignity, comfort, safety, and overall well-being.

Qualifications

• Current Illinois Licensed Practical Nurse (LPN) license required.

• Previous experience in a skilled nursing or long-term care setting preferred.

• Strong clinical, organizational, and communication skills.

• Ability to work effectively as part of a healthcare team.

• Compassionate approach to resident care and a commitment to quality service.

Schedule

• AM Shift: 7:00 AM – 3:00 PM

• PM Shift: 3:00 PM – 11:00 PM

• NOC Shift: 11:00 PM – 7:00 AM

• Rotating weekends required
