Champion Care Wisconsin is seeking an experienced, compassionate, and motivated Social Worker to support our skilled nursing and rehabilitation facilities throughout the state of Wisconsin.

This is a unique opportunity for a licensed social worker who enjoys working with multiple teams, providing guidance, solving problems, and making a difference in the lives of residents and their families.

In this role, you will serve as a resource and consultant to our facility social services teams, working closely with Administrators, Directors of Nursing, and interdisciplinary teams to strengthen social services programs, support regulatory compliance, and promote quality resident care.

Key Responsibilities

• Provide consultation, guidance, and hands-on support to social services departments across Champion Care's Wisconsin facilities.

• Assist facility teams with complex resident and family concerns, discharge planning, psychosocial assessments, and care coordination.

• Support the development, implementation, and improvement of social services policies, procedures, and best practices.

• Conduct facility visits to assess social services operations, identify opportunities for improvement, and provide recommendations.

• Assist with regulatory compliance, survey preparation, plans of correction, and follow-up activities related to social services.

• Provide training, mentoring, and ongoing education to facility social services staff.

• Collaborate with Administrators, Directors of Nursing, clinical leadership, and interdisciplinary teams to address resident needs.

• Support facilities with challenging discharge situations, resident rights concerns, grievances, and family communications.

• Assist with onboarding and orientation of new social services team members.

• Help establish consistent social services practices and documentation standards throughout the organization.

• Provide direct facility support when needed, including temporary coverage and assistance with special projects.

Qualifications

• Active Social Worker license required. Candidates licensed in Wisconsin or another state with an applicable Wisconsin licensure reciprocity or endorsement pathway are encouraged to apply.

• Degree in Social Work (BSW) or Master's degree in Social Work (MSW).

• Previous social work experience in a skilled nursing facility, long-term care, rehabilitation, or healthcare setting preferred.

• Knowledge of nursing home regulations, resident rights, discharge planning, and psychosocial services.

• Strong communication, organizational, and problem-solving skills.

• Ability to work independently while building positive relationships with facility leadership and staff.

• Experience with regulatory surveys, compliance, or multi-facility operations is a plus.

• Willingness and ability to travel throughout Wisconsin.
