As a Certified Activity Director at Horizon Post Acute, you will play a vital role in planning, organizing, and implementing a comprehensive program of activities and leisure pursuits designed to meet the social, spiritual, intellectual, emotional, educational, and physical needs and interests of our residents. Our ideal candidate will possess a strong passion for delivering high-quality care to our residents and is committed to providing exceptional programming that enhances their overall well-being.

Responsibilities:

• Develop and deliver activity/recreational programming to promote residents' opportunities for engaging in normal life
• Complete standardized assessment data for each resident, develop individualized plans of care, and implement strategies to assist residents with increasing their involvement in activities
• Plan community activities that jointly include the community, staff, families, and residents
• Provide residents with opportunities and support for involvement in group and individual leisure pursuits based upon individual interests and needs
• Assure physician's approval for involvement in activity/recreational programs and maintain precautions/limitations lists on each resident

Qualifications:

• At least 1 year of prior experience working at a LTC facility
• Accreditation as a Certified Activity Director and completion of a state-approved Activity Director training course
• Currently licensed as a Certified Nurses Assistant or willing to obtain license
