Position Overview

The CNA/HHA provides compassionate, personalized care to clients in the comfort of their homes. This role focuses on supporting clients with daily living activities, promoting independence, maintaining a safe home environment, and enhancing overall quality of life. Working under the direction of the Nursing Supervisor, the CNA/HHA delivers care in accordance with each client's individualized Plan of Care.

Key Responsibilities

• Personal Care: Assist clients with bathing, grooming, dressing, toileting, oral hygiene, and other activities of daily living.

• Mobility & Safety: Assist with ambulation, transfers, mobility devices, and maintaining a safe environment.

• Meal Preparation & Nutrition: Prepare meals, assist with eating, and support prescribed dietary needs.

• Client-Specific Care: Perform additional nurse-assigned tasks within the scope of training and authorization, which may include taking vital signs, assisting with range-of-motion exercises, and other approved care activities.

• Light Housekeeping: Complete routine household tasks such as laundry, dishes, dusting, sweeping, mopping, and vacuuming to maintain a clean and comfortable living environment.

• Companionship & Support: Build meaningful relationships with clients while promoting dignity, independence, and emotional well-being.

Professional Expectations

• Follow agency policies, procedures, and the client's established Plan of Care.

• Work under the supervision and direction of the Nursing Supervisor.

• Maintain client confidentiality, professionalism, and respect.

• Recognize and report changes in client condition, safety concerns, or other important observations.

• Perform only those services permitted by applicable regulations, training, and assigned duties.
