Position Overview

Always Best Care of Greater Cleveland is looking for a compassionate, reliable, and skilled PRN Physical Therapist to provide quality home health services to our clients.

This as-needed position is ideal for a Physical Therapist seeking flexible scheduling while helping patients improve mobility, strength, balance, and overall physical independence in the comfort of their homes.

Compensation Details:

• $53 per point therapy services. Mileage is paid at $0.43 per mile. Milage is not paid on travel for the first or last visit of your workday. OASIS assessments pay $45 per point. 

• OASIS Discharge is 1.56 points per visit 

• OASIS Non-Billable Discharge is 0.45 points per assessment 

• OASIS Recertification 1.56 points per visit 

• PT Discharge 1.5 points per visit 

• PT Resumption 1.5 points per visit 

• PT SOC 1.5 points per visit 

Responsibilities

• Evaluate patients' physical conditions, mobility, strength, and functional limitations.

• Develop and implement individualized physical therapy treatment plans.

• Provide therapeutic exercises and interventions to improve strength, balance, coordination, and mobility.

• Help patients improve their ability to perform daily activities safely and independently.

• Assess fall risks and recommend appropriate safety measures and assistive devices.

• Educate patients and caregivers on exercises, mobility techniques, and home safety.

• Monitor patient progress and adjust treatment plans when necessary.

• Complete accurate and timely clinical documentation.

• Coordinate with physicians, nurses, patients, families, and other healthcare professionals.

• Follow established care plans, clinical guidelines, and regulatory requirements.

Qualifications

• Graduate of an accredited Physical Therapy program.

• Current and valid Ohio Physical Therapist license.

• Previous home health experience preferred.

• Strong communication, organizational, and patient-care skills.

• Dependable transportation and willingness to travel to patients' homes.

• Ability to work independently and manage patient schedules.

Position Details

• Employment Type: PRN / Per Diem

• Schedule: Flexible, as-needed assignments based on patient census and availability.

• Service Area: Lorain, Elyria, Vermilion, Amherst, Wellington, Grafton, Strongsville, Avon Lake, and surrounding areas.
