Job Summary:

Upscale Home Care is seeking a compassionate and experienced Caregiver with SMI (Severe Mental Illness) experience to join our team of dedicated professionals. As a Caregiver with Upscale Home Care, you will have the opportunity to make a meaningful difference in the lives of our clients in the central Phoenix area, providing dignified support and care in the comfort of their own homes. Responsibilities:

• Provide one-on-one support and care to clients with SMI, including assistance with daily living activities, medication management, and behavioral support

• Develop and maintain positive relationships with clients, ensuring their physical, emotional, and social needs are met

• Collaborate with the interdisciplinary care team to create personalized care plans that respect each client's unique needs and goals

• Maintain accurate records and reports, documenting client progress and care provided

• Demonstrate a deep understanding of the importance of dignity, respect, and compassion in caregiving, upholding the values of Upscale Home Care

Requirements:

• 1 year of experience working with individuals with SMI, including experience with medication management, behavioral support, and daily living activities

• High School diploma or equivalent required

• Valid Arizona driver's license and vehicle or reliable transportation

• Strong communication and interpersonal skills, with the ability to build trusting relationships with clients and their families
