Job Summary:

Carease Health is seeking an experienced Nurse Practitioner (NP) with a background in Geriatrics or Internal Medicine to deliver high-quality preventative care to residents in skilled nursing facilities. As a hybrid role, you will combine on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation.

Responsibilities:

• Provide advanced nursing care to patients in skilled nursing facilities, focusing on proactive measures for enhanced physical health and well-being

• Conduct on-site visits and remote patient monitoring to detect abnormalities and prevent hospitalizations

• Collaborate with healthcare teams to develop individualized care plans, ensuring a dual-front effort in caring for both body and mind

• Maintain accurate and efficient documentation, supported by a dedicated scribe team

• Build strong relationships with facilities, patients, and families, maintaining a patient-centered approach

Requirements:

• Active NP license in the state of practice

• 1+ year of experience in Geriatric care and documentation review

• Excellent communication, collaboration, and teamwork skills
