As a MDS Coordinator at Avir at Park, you will play a vital role in ensuring the highest level of care for our residents. We are seeking a highly motivated and organized professional to join our dynamic team.

About the Role:

As a MDS Coordinator, you will be responsible for coordinating and conducting initial and comprehensive assessments, as well as quarterly assessments for our residents. You will work closely with our interdisciplinary team to develop and implement individualized care plans, ensuring that our residents receive the best possible care.

Responsibilities:

• Conduct initial and comprehensive assessments of residents, including ADLs, IADLs, and CPTs

• Develop and implement individualized care plans that align with resident goals and preferences

• Coordinate and conduct quarterly assessments, as well as assessments in response to changes in resident status

• Collaborate with the interdisciplinary team to identify and address resident care needs

• Maintain accurate and up-to-date records, including MDS forms, care plans, and assessments

• Communicate effectively with residents, families, and caregivers to ensure seamless care coordination

• Participate in quality improvement initiatives and strive for excellence in care delivery

Requirements:

• 0 years of experience in a healthcare setting (training or education may also be considered)

• High school diploma or equivalent required; degree in a relevant field (e.g., nursing, social work, or health science) preferred
