GoldLeaf Homecare NetworkMountain View, California
We're a woman-owned, locally rooted home care agency serving families across the Peninsula and South Bay. We grew through word of mouth, one family at a time and we continue to grow.
We support older adults, people living with dementia, and adults with developmental and intellectual disabilities. Our goal is to help each of them live safely, comfortably, and with dignity at home.
As a rapidly expanding organization and our continuous momentum, you'll have significant opportunities to grow your career and evolve alongside us. If you want work where you can see the difference you make every day, we'd love to meet you.
As our Client Care Manager, you'll be the heart of the care experience. You'll be the person families call first, the one caregivers trust, and the one who makes sure every client gets the right care from the right caregiver.
You'll guide each client's journey from the first phone call through ongoing care. That means assessing needs in the home, building personalized care plans, matching caregivers thoughtfully, and checking in often so small concerns never become big ones. You'll also help build and support our caregiver team: recruiting, onboarding, coaching, scheduling and recognizing the people who make our care possible.
You'll lead our caregiver team and client care operations. That means overseeing care plans, supervising and developing our caregivers, and owning the client and family experience from start to finish. The role combines time in our Mountain View office with regular client home visits, plus leading our on-call coverage every other weekend.
Conduct warm, thorough intake calls and in-home assessments to understand each client's physical, cognitive, emotional, and safety needs, along with their routines and preferences.
Create, update, and maintain personalized care plans that reflect each client's goals and changing conditions.
Serve as the main point of contact for clients and families. Give regular updates, answer questions, and help families understand how changes in condition affect care needs.
Spot changes early and escalate concerns appropriately, including coordinating with families, caregivers, and other providers when needed.
Complete supervisory and quality-assurance home visits and home safety checks to confirm care is safe, consistent, and meets GoldLeaf standards.
Supervise and direct our team of caregivers, including setting expectations, coaching performance, and recognizing great work.
Make hiring, discipline, and termination decisions and recommendations in partnership with the owner.
Oversee caregiver scheduling and client matching. You'll make the final call on the right caregiver for each client and resolve coverage gaps.
Review and approve caregiver timesheets and EVV records before payroll.
Lead caregiver recruiting: interview, screen, and select new caregivers, and give every candidate a positive experience.
Lead caregiver orientations, client-specific introductions, and in-service trainings.
Run caregiver performance reviews and decide on recognition, counseling, and corrective action in line with GoldLeaf policy.
Make sure personnel files for all caregivers stay complete and current.
Run our client satisfaction check-ins, follow up on concerns, and turn feedback into better care.
Listen to, investigate, and resolve client and family concerns with patience and care.
Build relationships with referral partners such as hospitals, rehab centers, senior living communities, and regional centers, and help with community outreach.
Connect families with helpful community resources.
Keep accurate, timely records of assessments, care plans, visit notes, incidents, and client communications in our care management system.
Follow California Home Care Services Bureau requirements, agency policies, and privacy standards.
Lead after-hours on-call coverage every other weekend. You'll handle caregiver call-offs, family questions, and urgent client needs. Weekend on-call is included in your salary but we’ll accommodate time off during the week when on-call duty is significant.
Have 3+ years of experience in care coordination, case management, or client services, including at least 3 year supervising or leading staff, ideally in home care, home health, hospice, assisted living, or a disability services setting.
Truly enjoy older adults, people living with dementia, and adults with developmental or intellectual disabilities, and treat every person with dignity.
Stay calm and clear-headed when things change quickly.
Communicate warmly and clearly, by phone, in person, and in writing, with families, caregivers, and healthcare partners.
Are highly organized, detail-oriented, and comfortable juggling many priorities.
Have good judgment and can work independently while knowing when to ask for help.
Are comfortable with technology, care management software and general office/computer tools. Experience with WellSky is an added advantage.
This is a hybrid office/field role with regular client home visits. A valid driver's license, reliable transportation, and auto insurance are required.
