Job Summary:

Carease Health is seeking an experienced Nurse Practitioner (NP) with a background in Geriatrics or Internal Medicine to deliver high-quality preventative care to residents in skilled nursing facilities. As a key member of our clinical team, you will play a crucial role in reducing hospitalizations and improving the lives of nursing home patients through remote monitoring, chronic care management, and psychiatric collaborative care.

About the Role:

As a Nurse Practitioner, you will be responsible for providing on-site visits to skilled nursing facilities, while also working remotely from home with the support of a dedicated scribe team for efficient documentation. Your success in this role depends on building strong relationships with the facility, maintaining a patient-centered approach, and being a collaborative team player.

Responsibilities:

• Provide high-quality preventative care to residents in skilled nursing facilities

• Conduct on-site visits and remote work from home, supported by a dedicated scribe team

• Maintain accurate and detailed medical records

• Collaborate with facility staff, healthcare providers, and family members to ensure comprehensive care

• Stay up-to-date with latest clinical guidelines and protocols

Requirements:

• Active NP or PA license (in the state of practice)

• 1+ year of experience in Geriatric care and documentation review

• Excellent communication, collaboration, and teamwork skills
