We are seeking an experienced Adult Nurse Practitioner to join our team at Carease Health - PA, a nationwide preventative healthcare company dedicated to reducing hospitalizations. As an Adult Nurse Practitioner with Carease Health, you will play a crucial role in delivering high-quality preventative care to residents in skilled nursing facilities.

About the Role:

As an Adult Nurse Practitioner with Carease Health, you will have the opportunity to work in a hybrid role that combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation. You will be working closely with the facility to build strong relationships, maintain a patient-centered approach, and be a collaborative team player.

Responsibilities:

• Provide high-quality preventative care to residents in skilled nursing facilities

• Conduct on-site visits and remote work from home

• Utilize remote patient monitoring (RPM), chronic care management (CCM), and collaborative care management (CoCM) to enhance patient care

• Collaborate with facility staff and Carease Health clinical team to ensure seamless integration of services

• Maintain accurate and timely documentation

• Participate in quality improvement initiatives to elevate patient care standards

Requirements:

• Active Adult Nurse Practitioner (NP) license in the state of Pennsylvania

• 1+ year of experience in Geriatric care or Internal Medicine

• Excellent communication, collaboration, and teamwork skills

• Ability to maintain confidentiality and handle sensitive information
