As an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine, you will deliver high-quality preventative care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation.

Responsibilities:

• Provide preventative care to residents in skilled nursing facilities

• Conduct on-site visits and remote monitoring using RPM, CCM, and CoCM

• Build strong relationships with facilities and maintain a patient-centered approach

• Collaborate with a dedicated scribe team for efficient documentation

• Participate in continuous quality improvement initiatives to enhance patient care

Requirements:

• Active NP or PA license in the state of practice

• 1+ year of experience in Geriatric care and documentation review

• Excellent communication, collaboration, and teamwork skills
