We are seeking a compassionate, experienced, and motivated RN Care Manager to participate in our continued growth. This is an opportunity to provide meaningful one-on-one care while building lasting relationships with clients and families in the comfort of their homes.

As an RN Care Manager, you will play a vital role in assessing client needs, coordinating care, supporting caregivers, advocating for clients, and helping individuals maintain their independence and quality of life.

Position Details

• Vero Beach, FL

• Part-Time with the opportunity to grow into Full-time if desired.

• $40–$45 per hour with opportunity for increases in wages.

• Schedule varies based on assigned clients' needs and the Care Manager's caseload; hours may include days, evenings, and minimal weekends

Qualifications

• Graduate of an accredited school of nursing

• Current Florida RN license

• Minimum two years of nursing experience; hospital or geriatric experience preferred

• Valid driver's license with clean driving record

• Reliable, insured vehicle

• CPR certification or willingness to obtain within 90 days of hire

• Strong communication, organization, and problem-solving skills

• Access to email, texting, and electronic documentation tools

Responsibilities

• Coordinate and oversee client care plans and documentation.

• Collaborate with clients, families, caregivers, and physicians

• Conduct assessments and monitor client progress

• Support clients with activities of daily living and health-related needs

• Ensure compliance with company policies and professional nursing standards

• Respond effectively to client needs and unexpected situations

• Participate in occasional on-call responsibilities as needed

• Coordinate and communicate with management and administrative team
