Nurse Practitioner

Rite Med Group is hiring a full-time Nurse Practitioner (NP) to care for the residents of AristaCare at Norwood Terrace, a 120-bed skilled nursing facility (SNF) in Plainfield, NJ offering short-term rehabilitation, long-term care (LTC), and memory care. As the Nurse Practitioner (NP), you will join a building with a 4.7-star Google rating across hundreds of family reviews, known for its dedicated rehab team and supportive leadership. The Nurse Practitioner (NP) position is the on-site clinical resource for the building, raising quality of care and reducing hospitalizations.

At least six months of Nurse Practitioner (NP) experience is required; prior nursing home experience is required.

Nurse Practitioner (NP) Duties

- Assess, diagnose, and treat short-term and long-term residents

- Evaluate new admissions and respond to changes in condition

- Order and interpret labs and diagnostics; manage medications

- Collaborate with attending physicians, the Director of Nursing (DON), and nursing staff

- Help reduce avoidable hospital readmissions

Nurse Practitioner (NP) Compensation and Schedule

- Pay: $150,000 per year

Nurse Practitioner (NP) Qualifications

- Active New Jersey Nurse Practitioner (NP) / Advanced Practice Nurse (APN) license

- Minimum six months of experience as a Nurse Practitioner (NP)

- Experience in a nursing home, skilled nursing facility (SNF), or long-term care (LTC) setting is required
