About the Role

As an MDS, you will be responsible for conducting comprehensive assessments of our residents' needs and developing individualized care plans to ensure their optimal health, comfort, and well-being. You will work closely with our interdisciplinary care team to identify and address resident needs, and communicate effectively with residents, families, and healthcare providers to ensure coordinated care.

Responsibilities

• Conduct comprehensive assessments of residents' physical, emotional, and social needs

• Develop and implement individualized care plans to meet residents' needs and promote optimal health and well-being

• Collaborate with the interdisciplinary care team to identify and address resident needs

• Communicate effectively with residents, families, and healthcare providers to ensure coordinated care

• Maintain accurate and up-to-date records of resident assessments and care plans

• Participate in quality improvement initiatives to enhance resident care and outcomes

Requirements

• Bachelor's degree in a related field (e.g. nursing, social work, occupational therapy)

• 5 years of experience in long-term care, gerontology, or a related field

• Current licensure or certification in a relevant field (e.g. Medicaid, Medicare)
