Licensed Practical Nurse (LPN) – Rehabilitation / Long-Term Care | Newark, NY

Join a 41-bed rehabilitation unit caring for adult and geriatric patients recovering from orthopedic and cardiac procedures, with opportunities to provide specialized wound care and manage complex clinical needs.

Assignment Details

• Location: Newark, New York

• Start Date: December 1, 2026

• Assignment Length: 13 weeks

• Shift: Mid shift, 2:00 PM – 10:00 PM

• Schedule: Five 8-hour shifts per week

• Weekend Requirement: Every other weekend

• Patient Ratios: Days 1:8, nights 1:20, weekends 1:10

• Floating: Required based on facility needs

• Local candidates: Not accepted; must reside more than 75 miles from the facility

Requirements

• Active New York LPN license required.

• Minimum of 1 year of LTC experience.

• Current BLS certification required.

• Previous Epic EMR experience required.

• Experience managing a patient ratio of approximately 1:10 required.

• Clinical skills in wound care, wound vacs, IV therapy, PICC lines, and nephrostomy care preferred.

• Experience caring for patients recovering from orthopedic and cardiac procedures, including those with hypertension, CHF, and renal failure.

• Must be willing to float according to facility needs.

• Requested time off must be disclosed at submission; candidates with more than 10 days of requested time off cannot be submitted.
