We are seeking experienced and compassionate Registered Nurses (RNs) to join our growing home care team as Home Care Case Managers. This is an excellent opportunity for nurses who are passionate about coordinating care, supporting patients in the community, and making a meaningful impact on patient outcomes.

Responsibilities

• Manage and coordinate patient care plans in a home care setting

• Conduct patient assessments and ongoing evaluations

• Collaborate with physicians, caregivers, and interdisciplinary teams

• Monitor patient progress and ensure quality care delivery

• Maintain accurate and timely clinical documentation

• Educate patients and families on treatment plans and health management

Qualifications

• Active Connecticut RN License

• Home Care or Hospital Case Management experience preferred

• Strong assessment, communication, and organizational skills

• Ability to work independently and manage multiple cases

• Bilingual candidates are encouraged to apply
