Responsibilities:

• Provide personalized in-home care services to clients, including assistance with daily living activities, mobility, and medication management

• Develop and maintain positive relationships with clients and their families to ensure their needs are met and exceeded

• Collaborate with our interdisciplinary team to develop and implement personalized care plans

• Maintain accurate and detailed records of client care and services provided

• Meet and exceed quality and safety standards to ensure a positive client experience

Requirements:

• 1 year of experience as a Home Health Aide (HHA) or equivalent

• Ability to lift, transfer, and position clients as needed

• Reliable transportation
