Seeking an experienced Registered Nurse for a Nurse Assessor opportunity supporting home and community-based care in Queens, NY. This role involves conducting comprehensive patient assessments, evaluating long-term care needs, and coordinating services to help consumers receive appropriate care and support. Hybrid telephonic and in-field assessments are required.

Requirements

• Active New York State RN license.

• At least two years of qualifying home-based care experience within the last 10 years.

• Recent experience conducting comprehensive patient assessments.

• Experience in a certified home health agency, licensed home care agency, managed long-term care, OASIS assessments, or similar qualifying home-based care settings.

• Ability to conduct both telephonic and in-field assessments.

• Strong clinical judgment, documentation, communication, and organizational skills.

• Ability to work independently in a fast-paced environment.

• BLS certification required before the start date if not already available.
