As a compassionate and dedicated Mental Health Provider, you will play a vital role in our Collaborative Care team, providing emotional support, brief therapeutic interventions, and mental health assessments to residents in skilled nursing facilities. At Carease Health - TN, we prioritize proactive measures for enhanced physical health and well-being, incorporating Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM) into our comprehensive preventative care program.

About the Role

We are seeking a skilled and empathetic Care Companion/Social Worker to join our team. In this part-time position, you will work closely with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP. Your responsibilities will include:

• Visiting designated facilities in accordance with the schedule set with your supervisor

• Conducting assessments of residents to identify emotional, cognitive, and psychosocial needs

• Implementing short-term, solution-focused therapy and supportive interventions to address immediate concerns

• Documenting visit notes and maintaining detailed records of interactions and interventions after each visit

• Collaborating with psychiatric professionals to ensure comprehensive care

Qualifications

• Associates, Bachelor's or Master's degree in Social Work, Psychology, Counseling, or another mental health–related field

• Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings

• Strong communication, documentation, and relationship-building skills

• Education: Associate Degree

• Experience: 1 year
