The Greens at Pinehurst is seeking an experienced, detail-oriented RN/LPN MDS Coordinator to join our team!

We're looking for a dedicated nursing professional with strong knowledge of the MDS process, care planning, and regulatory compliance who takes pride in ensuring accurate assessments and quality resident care.

KEY RESPONSIBILITIES

Coordinate and oversee the RAI/MDS assessment process, ensuring accuracy, completeness, and timely submissions
Monitor completion of MDS assessments, CAAs, and interdisciplinary care plans
Ensure accurate transmission of MDS assessments and obtain validation reports
Schedule and facilitate interdisciplinary care plan conferences
Participate in monthly Quality Measure (QM) meetings
Manage the care planning process while maintaining regulatory compliance
Collaborate with nursing leadership to support quality resident outcomes

QUALIFICATIONS

Active, unrestricted RN or LPN license in North Carolina
Graduate of an accredited nursing program
Minimum 3 years of MDS Coordinator experience in Long-Term Care
Strong knowledge of MDS assessments, Case-Mix, Medicare and Medicaid reimbursement
Familiarity with federal and state regulations governing skilled nursing facilities
Excellent organizational, communication, and critical-thinking skills
RAC-CT Certification Preferred
