Licensed Practical Nurse (LPN) – Full-Time
12-Hour Shifts Available (Days)

Millennium Post Acute Rehabilitation is a 132-bed skilled nursing and rehabilitation facility in West Columbia, SC, looking for dedicated LPNs to join our team.

Responsibilities

• Provide direct patient care and monitor resident conditions

• Administer medications and treatments per physician orders

• Perform routine procedures (vitals, injections, wound care, etc.)

• Document accurately in patient charts

• Communicate changes in condition to physicians and the nursing team

• Collaborate on individualized care plans

• Support residents’ daily needs in a skilled nursing / post-acute setting

Requirements

• Active South Carolina LPN License (required)

• Experience in skilled nursing, long-term care, or post-acute preferred

• Ability to work 12-hour shifts (days or nights)

• Strong clinical and documentation skills
