About the Role:

• As a Part Time Night LPN or RN, you will be responsible for delivering high-quality patient care to our residents, ensuring their physical, emotional, and psychological needs are met.

• You will work collaboratively with our interdisciplinary team to develop and implement individualized care plans, addressing the unique needs of each resident.

• Your excellent communication skills and ability to work effectively with families, physicians, and other healthcare professionals will be essential in ensuring seamless care transitions and achieving optimal outcomes.

Responsibilities:

• Conduct initial assessments and ongoing evaluations of residents' physical, emotional, and social conditions.

• Develop and implement individualized care plans, incorporating resident goals, preferences, and needs.

• Administer medications, treatments, and therapies as prescribed by physicians and as outlined in care plans.

• Monitor and report changes in residents' conditions, ensuring prompt notification of physicians and other healthcare professionals.

• Provide emotional support and comfort to residents, their families, and other caregivers.

• Maintain accurate and timely documentation of resident care, including progress notes, treatment records, and medication administration records.

Requirements:

• Current licensure as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) in the state of Missouri.

• High school diploma or equivalent required; associate's or bachelor's degree preferred.

• Ability to work varying shifts, including nights and weekends, as required.

• Excellent communication, organizational, and problem-solving skills.
