Job Summary:

We are seeking a compassionate and organized Hospital Liaison to join our team at Elizabethtown Nursing and Rehabilitation, a premier skilled nursing and rehabilitation center located in Elizabethtown, PA. As a Hospital Liaison, you will play a vital role in ensuring seamless transitions between our center and acute care hospitals. Responsibilities:

• Collaborate with hospital staff to coordinate patient care and transfers, ensuring a smooth transition from hospital to rehabilitation center

• Develop and maintain positive relationships with acute care hospitals, rehabilitation centers, and other healthcare providers

• Identify and address potential barriers to patient care, working closely with interdisciplinary teams to resolve issues

• Ensure compliance with regulatory requirements and maintain accurate records of patient data and hospital interactions

• Participate in quality improvement initiatives to enhance patient outcomes and satisfaction

Requirements:

• 1-2 years of experience in a healthcare setting, preferably in a liaison or coordination role

• High school diploma or equivalent required; degree in healthcare-related field preferred

• Excellent communication, organizational, and problem-solving skills

• Ability to work in a fast-paced environment and prioritize tasks effectively

• Strong attention to detail and ability to maintain accurate records
