About the Role:

We're seeking a dedicated LVN to join our team as a Charge Nurse, providing clinical care to our residents in our skilled nursing facility. As a Charge Nurse, you'll be responsible for leading and supporting our care team, as well as mentoring new staff and sharing your knowledge with team members.

Responsibilities:

• Provide direct nursing care in accordance with resident care plans, physician orders, and community policies.

• Administer medications and treatments accurately and on time.

• Monitor resident conditions and intervene as needed to ensure health and safety.

• Support CNAs and other direct care staff to ensure consistent, quality care.

• Communicate changes in resident status to physicians, families, and interdisciplinary team members.

• Participate in care planning, resident assessments, and quality improvement initiatives.

What We're Looking For:

• Active LVN license in the state of Texas.

• CPR certification required (or willingness to obtain upon hire).

• Previous experience in skilled nursing or senior living preferred, but not required.

• Strong assessment, documentation, and communication skills.

• Ability to stay organized and calm under pressure while leading others.

• Ability to lift, move, and assist residents as needed.

• Must be able to stand, walk, bend, and perform physical tasks throughout a full shift.
