We are seeking a dedicated and compassionate LPN Licensed Practical Nurse to join our team at Rutland Center for Living and Rehabilitation in Rutland, Vermont. As a valued member of our healthcare team, you will play a critical role in providing exceptional patient care and support.

About the Role:

As an LPN Licensed Practical Nurse, you will be responsible for delivering high-quality patient care in a long-term rehabilitation and subacute care setting. You will work collaboratively with our interdisciplinary team of healthcare professionals to develop and implement individualized care plans, monitor patient progress, and provide direct patient care.

Responsibilities:

• Provide direct patient care, including administering medications, wound care, and other treatments as ordered by physicians
• Monitor vital signs, report changes in patient condition, and document patient information in electronic medical records
• Assist with daily living activities, such as bathing, dressing, and grooming
• Participate in patient assessments, develop and implement care plans, and provide education to patients and families
• Collaborate with other healthcare professionals to coordinate patient care and achieve desired outcomes
• Maintain a clean and safe environment, adhering to infection control protocols and facility policies

Requirements:

• 2 to 10 years of experience as an LPN Licensed Practical Nurse
• Current LPN Licensed Practical Nurse licensure in the state of Vermont
• Strong communication and interpersonal skills
• Ability to work effectively in a fast-paced environment
• Familiarity with electronic medical records and computerized documentation
