Help Residents Build Independence

Skilled Nursing and Rehabilitation Center is seeking a compassionate and experienced Occupational Therapist (OT) to join our rehabilitation team. In this full-time role, you’ll help residents improve daily living skills, regain independence, and achieve their highest level of function.

Key Responsibilities

• Evaluate residents’ functional abilities and develop individualized treatment plans based on their needs and goals.

• Provide occupational therapy services that support independence, safety, and participation in daily activities.

• Monitor resident progress and adjust treatment plans as appropriate.

• Collaborate with physicians, nursing staff, rehabilitation professionals, and other interdisciplinary team members.

• Participate in resident assessments, care plan meetings, and discharge planning.

• Educate residents and families on strategies to support functional progress and a safe transition after discharge.

• Maintain accurate and timely evaluations, treatment notes, progress reports, and billing documentation.

• Follow established therapy standards, facility policies, and applicable regulatory requirements.

• Support the Rehabilitation Director with clinical initiatives and departmental improvements.

• Participate in quality assurance and performance improvement activities.

• Maintain a compassionate, supportive, and resident-centered therapy environment.

Qualifications

• Active Florida Occupational Therapist license.

• Minimum of one year of occupational therapy experience in a skilled nursing facility.

• Strong knowledge of skilled nursing and rehabilitation practices.

• Excellent communication, organizational, and documentation skills.

• Ability to collaborate effectively with an interdisciplinary healthcare team.
