About the Role:

As an LPN Licensed Practical Nurse at Lock Haven Rehabilitation & Senior Living, you will be responsible for providing direct patient care, administering medications, and supervising Certified Nursing Assistants (CNAs). You will work closely with the interdisciplinary care team to develop and implement individualized care plans, ensuring that each patient receives the highest level of care.

Responsibilities:

• Provide direct patient care, including administering medications, wound care, and other treatments as needed
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Monitor patient vital signs and report any changes to the nursing team
• Supervise Certified Nursing Assistants (CNAs) and provide guidance and support
• Collaborate with other healthcare professionals to ensure seamless care transitions
• Document patient care and treatment plans accurately and thoroughly
