Responsibilities:

• Assess, plan, and implement individualized physical therapy programs to meet the unique needs of our residents

• Conduct thorough evaluations and develop comprehensive treatment plans to improve mobility, strength, and overall function

• Implement and document patient treatments, ensuring accurate and timely documentation

• Collaborate with other healthcare professionals to develop and implement interdisciplinary care plans

• Monitor and report patient progress to family members and healthcare team members

• Maintain accurate and up-to-date records and reports as required

Requirements:

• Bachelor's or Master's degree in Physical Therapy from an accredited institution

• Valid Kansas license as a physical therapist

• Minimum of 0 years of experience in a skilled nursing facility (SNF) or related healthcare setting

• Strong communication and interpersonal skills

• Ability to lift and transfer patients as needed
