Social Worker Responsibilities:

• Assess the social, emotional, and psychological needs of our residents, providing personalized support and interventions as needed

• Develop and implement comprehensive care plans in collaboration with the interdisciplinary team

• Facilitate connections with community resources, services, and organizations to support residents' well-being and independence

• Provide emotional support and counseling to residents, families, and staff members

• Document resident interactions, assessments, and treatment plans in accordance with established policies and procedures

• Participate in quality improvement initiatives and contribute to the development of best practices in social work services

Social Worker Requirements:

• MSW preferred, Bachelor's degree in Social Work (BSW) also accepted

• Licensure as a Social Worker in the state of New York (LSW or LCSW)
