About the Role

As an LPN at Woodlawn, you will:

• Provide nursing care, medications, and treatments according to residents’ care plans and provider orders.

• Monitor residents and communicate changes in condition to the appropriate clinical team members.

• Maintain accurate and timely documentation.

• Collaborate with nursing assistants and the interdisciplinary team to support safe, personalized care.

• Communicate respectfully with residents and their families.

Qualifications

• Graduate of an approved practical nursing program.

• Current authorization to practice as an LPN in New Hampshire, with a license in good standing.

• Commitment to compassionate, resident-centered care.

• Skilled nursing or long-term care experience preferred.
