About the Role of LPN for Weekends:

We are seeking a dedicated and skilled LPN to join our weekends team. As a LPN, you will be responsible for providing direct patient care, administering medications, and performing wound care, vital signs, and other treatment modalities. You will work closely with our interdisciplinary team to develop and implement personalized care plans that meet the unique needs of our residents.

LPN for Weekends Responsibilities:

• Provide direct patient care to residents in a sub-acute and long-term care setting

• Administer medications and treatments as prescribed by physicians

• Perform wound care, vital signs, and other treatment modalities

• Develop and implement personalized care plans in collaboration with the interdisciplinary team

• Document patient care and progress accurately and timely

• Participate in Quality Improvement initiatives to enhance patient care and satisfaction
