Deer Meadows Rehabilitation Center is seeking a dedicated and compassionate LPN Licensed Practical Nurse for SNF Weekends to join our team. As a key member of our skilled nursing facility (SNF) team, you will play a vital role in providing high-quality, patient-centered care to our residents. LPN for SNF Responsibilities:

• Provide direct care to residents in our SNF setting, including administering medications, wound care, and vital signs

• Develop and implement individualized care plans to meet the unique needs of each resident

• Collaborate with interdisciplinary teams to ensure comprehensive care and coordination

• Monitor and document resident status, reporting any changes or concerns to the healthcare team

• Participate in quality improvement initiatives to enhance resident outcomes and satisfaction

• Maintain accurate and thorough records of patient care

• Provide education and support to residents, families, and caregivers

LPN for SNF Requirements:

• LPN License in the state of Pennsylvania

• Strong communication and interpersonal skills

• Ability to work effectively in a fast-paced, dynamic environment
