Shady Nook Nursing & Rehab is seeking a compassionate and motivated Physical Therapist Assistant (PTA) to join our therapy team on a PRN basis. This is a great opportunity for a PTA looking for flexibility while making a meaningful difference in the lives of residents.

Position Overview

The Physical Therapist Assistant will work under the direction and supervision of a licensed Physical Therapist to provide skilled therapy services to residents in a skilled nursing and rehabilitation setting. The PTA will implement individualized treatment plans while promoting resident progress, safety, mobility, and quality of life.

Key Responsibilities

• Provide skilled physical therapy treatments under the direction and supervision of a licensed Physical Therapist

• Implement individualized treatment plans based on resident needs and established goals

• Assist residents with therapeutic exercises, mobility training, transfers, balance, and functional activities

• Monitor and document resident progress and response to treatment

• Communicate effectively with the Physical Therapist, nursing staff, residents, families, and other members of the care team

• Maintain accurate and timely clinical documentation

• Promote resident safety, independence, and quality of life

• Participate in interdisciplinary care planning and team meetings as appropriate

• Maintain compliance with applicable state and federal regulations and facility policies

• Provide compassionate, resident-centered care in a professional and supportive environment

Qualifications

• Current Physical Therapist Assistant (PTA) license in the state of Indiana

• Graduate of an accredited Physical Therapist Assistant program

• Current CPR certification or ability to obtain

• Previous experience in skilled nursing, rehabilitation, or long-term care preferred

• Strong communication and interpersonal skills

• Ability to work independently and effectively as part of an interdisciplinary team

• Dependable, compassionate, and committed to providing quality resident care
