At Unity Health and Rehabilitation Center, we are committed to providing exceptional care and services to our residents, and we are seeking a dedicated and experienced Director of Social Services to lead our social services department. As a key member of our management team, you will play a vital role in ensuring the well-being and quality of life for our residents.

About the Role:

As the Director of Social Services, you will be responsible for overseeing the development, implementation, and evaluation of our social services program. You will work closely with our medical team, therapy staff, and other departments to ensure that our residents receive comprehensive care and support. You will also be responsible for:

• Developing and implementing social services programs to meet the unique needs of our residents

• Recruiting, training, and supervising social services staff

• Monitoring and evaluating the effectiveness of our social services program

• Collaborating with our therapy team to develop and implement individualized treatment plans

• Ensuring compliance with regulatory requirements and accreditation standards

Responsibilities:

• Develop and implement social services programs to meet the needs of our residents

• Recruit, train, and supervise social services staff

• Monitor and evaluate the effectiveness of our social services program

• Collaborate with our therapy team to develop and implement individualized treatment plans

• Ensure compliance with regulatory requirements and accreditation standards

• Participate in quality improvement initiatives to improve resident outcomes

• Communicate with residents, families, and healthcare providers to ensure seamless coordination of care

Requirements:

• Master's degree in Social Work

• At least 2 years of experience in social services administration or management

• Strong leadership and communication skills

• Ability to work effectively with diverse populations

• Familiarity with regulatory requirements and accreditation standards

• Ability to work varying shifts, including weekends and holidays
